CONSENT TO PROCESSING OF THE SPEAKER'S PERSONAL DATA
I hereby consent to the processing of my personal data in accordance with Article 6(1)(a) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (General Data Protection Regulation - RODO), with regard to: publication of my name, title/degree and name of the parent unit, topic and title of the speech, function and name of the institution Lublin University of Technology for the purpose of posting a photo-report from the Scientific Conference “PROTECHMA 2025” on the website of the Lublin University of Technology. PROTECHMA 2025 on the website, in publications, post-conference materials and publications, and through all other media/information distribution channels, on the PL website and news and press materials in connection with my participation in the conference.
CONSENT TO PROCESSING OF THE SPEAKER'S IMAGE
Pursuant to Article 81 paragraph 1 of February 4, 1994 (Journal of Laws of 2006, No. 90, item 631, as amended) on Copyright and Related Rights, I hereby declare that I agree for free, repeated dissemination of the recording of my image on websites, in regional media, in publications and through any other media/channels of information distribution only and exclusively for informational, educational, promotional and advertising purposes related to the realization of the Scientific Conference “PROTECHMA 2025”. PROTECHMA 2025 organized by Lublin University of Technology may be used for various forms of electronic image processing, framing and composition, without obligation to accept the final product, but not in forms that are offensive or generally considered unethical.
At the same time, I declare that the photos and videos do not violate my personal rights.
This consent:
- It is not limited in time or territory.
- Applies to recordings with my participation, related to the realization of information and press materials, publications, documentation for AZ related to the realization of the Scientific Conference entitled PROTECHMA 2025.
Information clause on the processing of personal data.
Pursuant to Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016.
on the protection of natural persons with regard to the processing of personal data and on the free flow of such data and repealing Directive 95/46/EC (hereinafter referred to as “RODO”) (Official Journal of the EU L 119 of 04.05.2016, p. 1), informs that:
1) the controller of your personal data provided in the registration form is Lublin University of Technology, Nadbystrzycka 38D Street, 20-618 Lublin;
2) the personal data protection inspector at Lublin University of Technology is Mr. Tomasz Joński, e-mail contact: iod@pollub.pl (the data protection officer can be contacted in all matters concerning the processing of personal data and the exercise of rights related to data processing);
3) personal data provided in the registration form are processed for the purpose of registering participation in the PROTECHMA 2025 Conference, organized on June 25-27, 2025 by the Department of Metal Forming, Faculty of Mechanical Engineering, Lublin University of Technology, in organizing and communicating with participants on matters related to the organization of the conference, documenting its course, including drawing up a list of participants, issuing certificates of participation, archival purposes. The legal basis for the processing of personal data in the following areas: image, name, university - place of work, title of the paper is Article 6(1)(e) of the RODO.
You may withdraw your consent at any time - which does not affect the legality of the processing performed before its withdrawal;
4) the personal data you provide will not be disclosed to other entities, except to those authorized by law, including those involved in the organization or operation of the conference;
5) personal data processed in connection with your participation in the conference will be processed for the period of fulfillment of the purpose of processing, as well as for the period of retention of financial and accounting records of the conference by the university; in situations provided by law, they may also be processed for the period necessary to establish and pursue possible claims. In the case of consent to receive information about conferences, symposia and events with a scientific profile or popularizing science, personal data will be processed until the consent is withdrawn or an objection to the processing is expressed.
6) The data subject has the right to access the content of his/her personal data and the right to rectify, erase or restrict their processing, the right to data portability, the right to object to their processing, the right to withdraw consent at any time without affecting the lawfulness of processing carried out on the basis of consent before its withdrawal;
7) The data subject has the right to lodge a complaint with the supervisory authority - the President of the Office for Personal Data Protection, if he/she considers that the processing of personal data concerning him/her violates the provisions of the General Data Protection Regulation;
8) with regard to your personal data, decisions will not be made by automated means, nor will they be subject to profiling,
9) the data provided will not be shared with third countries and international organizations;
10 )the event will be photographed for documentation and promotional purposes of Lublin University of Technology, hereinafter referred to as the “Organizer”. Participation in the Event is tantamount to consent to the use of image in photographic materials.
11) Provision of data is voluntary, but necessary for the purposes for which they were collected, without them it will not be possible to participate in the conference.
